IMPORTANT: Information will not be verified over the phone, DIPLOMAS CANNOT BE DUPLICATED & Official Transcripts cannot be faxed.  An Official Transcript is considered official documentation of graduation by colleges and employers.   *Parent or legal guardian may request records if the student is under 18 years old but must include a copy of valid ID as well as the student ID.  Transcript requests will not be processed until request form, payment, and a copy of IDs are received.  We process requests in the order in which they are received and it might take up to 5 working business days after all requirements are received to process requests. Expect longer delays at the end of the school year.  If requesting via mail, allowed transit time for your request to arrive at us. *All Official Transcripts are sent via USPS.  If the former student graduated from Adult School, Phoenix Program, or another school, you must contact the corresponding office since those records are not kept at MMHS. Records other than transcripts are kept on file only for 5 years after graduation or last date of attendance; records are destroyed after 5 years for data security.

The request must include:
 
1- A signed Release of Information from the person you are requesting information,  
2- Full legal name used while attending Mount Miguel High School (Maiden name)
3- Year of graduation or last year of attendance
4- Full date of birth (Month / Day / Year)
5- Fax request (3 pages max) to 619-644-8408, emails are NOT accepted.

Requests missing information will not be processed

 
 
 
 

Transcripts & Other Records

 
The request must be sent via mail and must include:
 
1- A signed Release of Information from the person you are requesting information (3 pages max)
2- Full legal name used while attending Mount Miguel High School (Maiden name)
3- Year of graduation or last year of attendance
4- Full date of birth (Month / Day / Year)
5- Payment in the exact amount, $2.00 per transcript/record.
   *We only accept money orders, money grams, or cashier checks.
